Some companies in Texas require an application fee while others don't.  For instance, UniCare has an application fee, but if you apply online the fee is waived.  On the other hand, BlueCross BlueShield of Texas charges an application fee and automatically deducts the first month's premium from your account.  If your application is declined, they refund your first month's premium, but not your application fee.  UnitedHealthcare has no application fee, but they require the first month's premium up front as well.  Humana has no application fee and only deducts your first month's premium when you are accepted for coverage.
